As a Through The Trees (TTT) participant or TTT participant legal guardian, I have informed the TTT Staff of any medical needs I/my child may have and give the Staff permission to administer basic first aid and prescription medicines as needed and directed. While I/my child am/is participating in any TTT activity, I take full responsibility for any physical injuries or conditions that I/my child may have at this time or have had in the past and I relinquish the rights to sue. Through The Trees, its Staff, and associated people will not be held liable for any injuries, illnesses, or damages that may occur during the course of these programs or events or during transportation to and from events and/or locations.
Assumption of Risk of physical injury. By engaging in these Services I am aware that I agree to fully accept all known and unknown risks, including any injury, harm or death. I understand and agree to hold Through The Trees harmless and not liable for any injury or damage that I may sustain while using their services.
Consent to First-Aid or Emergency Treatment. I consent to the application of first-aid or other medical or mental health services to be applied if needed in connection with an emergency health problem or potentially harmful situation during the Services, and I agree to hold Through The Trees harmless as a result of any such injury or damage suffered due to the application of medical or mental health services or treatment. I also agree and consent that Through The Trees may contact my Emergency Contact as shown on the bottom of this form and share detailed information about the emergency without violating my privacy rights.
Assumption of Risk of Exposure to COVID-19. By engaging in these Services, I am aware that I agree to fully accept all known and unknown risks, including the potential risk of exposure to respiratory illnesses or other illnesses, diseases, or conditions, including but not limited to the coronavirus known as COVID-19. Although the Through The Trees staff is complying with state regulations, executive orders by the governor, federal laws, local laws, and CDC guidelines regarding cleaning, disinfecting and practices which reduce the potential for exposure to COVID-19, I understand that my child(ren) and/or I may be exposed to COVID-19 or its symptoms through no fault of the Through The Trees. I understand and agree to hold Through The Trees harmless and not liable for any real or perceived Symptoms of COVID-19 or any other disease, illness, or condition, nor for exacerbating any existing Symptoms of any illness, disease or condition, and I fully agree to accept these and all other known and unknown risks of receiving Services from the Through The Trees.
Assumption of Risk of Exposure to Tick-Borne Illness or other Insect-related Injury or Illness. By engaging in these Services, I am aware that I agree to fully accept all known and unknown risks, including the potential risk of Lyme and other Tick-Borne Illness. I understand and agree to hold Through The Trees harmless and not liable for any real or perceived Symptoms of any Tick-Borne Illness, insect sting or bite and I fully agree to accept these and all other known and unknown risks associated with ticks and other insects.
I have carefully read this document and by signing below I consent to all parts of it for myself and/or my children and family participating with me. I understand that by signing this Release and Waiver, I voluntarily surrender certain legal rights.
